• Access to educational documentation regarding your condition or procedure
• Or access to form for health history or as needed for upcoming clinical procedures.

To register, you will need your first and last name, the email address you used when setting up an account with Riordan Clinic, and your birthdate.  This data is usd to verify the account belongs to you.  Once registered, the email address will be used to notify you of secure messages, and other items related to your care.  We are committed to protecting your personal information. Data that you provide cannot be viewed by anyone else on the Web and is securely maintained by industry standard security.

We do not share or sell your information to other parties. To help Riordan Clinic keep your electronic personal health information (ePHI) secure, do not share your Username or Password with other parties.  

Parents or guardians, please contact Riordan Clinic prior to registering for access to a child's or dependant's records.  We will need to ensure we have your appropriate contact information associated with that of your dependant prior to registering.  You may be required to show proof of guardianship.
